Transient Synovitis of Hip

일과성 고관절 활액막염

임상소견

• 동통, 바이러스감염이나 가벼운외상 선행되기도
• 서혜부, 고관절부 동통과 파행
• 수일에서 수주까지 지속
• 평균 10일 대개 4주이내 소실

검사소견

• WBC, ESR 약간 증가할 수도, 대체로 normal
• Legg-Calve-Perthes 병과 감별이 중요 -> MRI에서 capital epiphysis necrotic lesion에 low signal intensity
• DDx. LCP disease, 각종 Arthritis(septic, TB, JRA etc.)
  LCP - epiphysis간 간격이 넓어지고 골단 상부의 외연이 약간 편평성(flattening) metaphysis 횡경 좁아짐

치료

• 동통 소실시까지 체중부하 금하고 침상 안정
• 관절운동회복(3-7일)후에도 2주정도 체중부하 금지
• NSAIDs
• traction - 가급적 안한다.

예후: 4-6%재발(대개 6개월 이내) LCP병은 대개 2-6개월에 진단되므로 이 시기에 주의

HIVD (Herniated Intervertebral Disc)

• 30-50세 사이 남자에 호발
• L4-5, L5-S1에서 호발
• 후측방60% 정후방30% 측방 10%, 원측방(farlateral)은 더 고령
• 외상과 연관 - 무거운 물건 들 때 허리에서 뚝하는 느낌 (snapping sensation), 허리 굽힐때 걸리는 느낌 (catching sensation)
• 주증상: 요통 및 방사통
  천장관절, 둔부, 서혜부, 대퇴부로 방사되는 관련통
  이환된 신경근을 따라 방사되는 대퇴 신경통 및 좌골 신경통
• 관련통은 주로 양측 하지의 근위부로 방사, 무릎이하로 잘 안감
• 신경통은 무릎 지나 발끝까지 방사
• S1 root: 족부굴곡근 근력약화, 발끝보행 어려움, DTR 이상
• L4-5: 슬개건 아킬레스건 반사 변화없음,
  좌골신경통이 심할 때 좌골신경 주행따라 압통유발(Valleix point)
  SLR test - 발끝까지 방사되는 심한 동통, 30-70도에서 양성이면 HIVD 의심
• Bowstring test: SLR 후 무릎 굽혀 동통 없앤 후 무릎 뒷편 좌골신경 지나가는 부위 누를 때 방사통 다시 나타남
• Bragard test: reinforced SLRT: SLR후 동통오면 하지 약간 내리고, 발목을 뒤로젖힘
• Kernig test: 목을 숙임
• Well leg raising test: Crossed SLRT 건측다리 올려 검사할 때
• 진단: plain X-ray로는 normal lordotic curvature가 감소, CT MRI 등의 검사
  

치료

• 자연적으로 증상이 없어질 때까지 얼마나 환자를 편안하게 하는가가 보존적 치료의 원칙
• 보존적 치료: 절대안정, NSAIDs, P-traction, hot pack, ultrasound, TENS, 보조기, 복근강화
• 수술적 치료

경추 신경근증

연성추간판질환(soft disc) 30-40대 호발, 추간판의 탈출

경성추간판질환(hard disc) 50대 이후에 호발, 구추관절(uncovertebral joint) osteophyte에 의해 신경압박

병변부위: C5-6, C6-7 이 m/c 그다음 C4-5

1. 증상 및 징후: 초기 증상은 경추부 동통과 강직 및 원위부의 감각 이상
   연성인 경우 증상이 갑자기 생기고 상지 외전시 동통경감(shoulder abduction relief sign)
   경성인 경우 서서히 증상 생기며 상지외전으로 경감없음
2. C4-5: 5th cervical root 압박, 상박 외측 주관절 외측 감각이상, 삼각근 및 상완 이두근 근력약화
3. C5-6: 6th root 압박, 전박 외측 제1,2 수지 감각 이상, 상완이두근 및 장단요수근신근(extensor carpi radialis longus/brevis) 근력약화
4. C6-7: 7th root 압박, 제3수지 감각이상, 삼두근, 요수근굴근(flexor carpi radialis)등의 근력 약화
5. DDx. peripheral nerve entrapment syndrome, rotator cuff/shoulder pathology, brachial plexitis, herpes zoster, thoracic outlet syndrome, sympathetic mediated pain syndrome, tumors, abscess, cardiac ischemia
6. Tx. conservative therapy (bed rest, hot packs) 편안한 위치 유지 등
7. Spurling sign - 목 신전 환측으로 회전하면 환측 상지로 동일한 방사통이 유발
   
8. Compression test - vertex 압박하면 환측 상지로 방사통 유발

My Opinion

It's a kind of hot potato. Recently one female elderly who was in coma state was judged to come to death naturally. Her mechanical ventilator was removed under the judical decision, but she is still alive without any mechanical help.
I don't think any human can decide the human's death. I don't agree with Euthanasia nor Death penalty. The only thing that doctors can do for those who want to die is to relieve their physical and mental pain. Some of their medication and verbal actions can help them. And I think all humans have rights to live without pain; doctors can help them do so. Doctors should use pain relievers enough for patients in terrible pain, even though the patients lose their consciousness or die because of the pain killers. But it is limited within the terminally ill patients.
More discussions and more time will be needed to establish the social agreement.
